Once your primary care Physician has sent in a request for you to be assessed, a Registered Nurse will contact you from the state of North Carolina to schedule your initial assessment. During the assessment the state nurse will decide if you meet the criteria for Medicaid PCS services and what level of care will be most beneficial to meet your needs. The state RN will also asses what you can and can not do for yourself by observing you attempting different tasks. Once your assessment is completed, the state RN will ask what Home Care Agency you would like to provide your home care services. When we receive your information, Able Home Health Care’s Registered Nurse will review all of your information and contact you to schedule a visit to further assess your needs and develop the plan of care. Once the plan of care has been established a personal caregiver will be paired with the client who best fits their needs. Our RN will continue to monitor their care and check in every 90 days to ensure that their needs and concerns are being met.
Able Home Health Care In-Home Caregivers are carefully screened, interviewed and trained. We like to take a personal approach when hiring and staffing. Our caregivers are understanding, nurturing, skilled and experienced in caring for seniors, other adults and children. Every caregiver that Able Home Health Care employees has a current license and/or certificate (where applicable), a clear criminal background check, a health care registry check in accordance with North Carolina state law. In addition, all of the caregivers are required to provide a comprehensive work history, including verified personal and professional references. It is most important to us to put your needs first. We will only hire a caregiver that has the necessary experience and disposition to fulfill your individual needs.
